Mr S Anderson asked the Minister of Health, Social Services and Public Safety for an update on the Policy Framework, Living with long term conditions in improving the quality of care for people living with chronic conditions. (AQO 2087/11-15)
Mr Poots provided the following written answer between 28 May and 1 June in Northern Assembly:
"On 26 April I launched my Department’s Policy Framework, Living with long term conditions. The Framework will help drive the modernisation of health and social care services for adults living with long term conditions and their carers, supporting my vision of driving up the quality of care, improving outcomes and enhancing the patient experience.
"The Framework sets out clear principles and good practice for commissioners and providers in the statutory, independent, voluntary and community sectors who are responsible for planning and
delivering treatment and care to support people living with long term conditions.
"Importantly, the Framework reflects the new model of care recommended in Transforming your care, which includes long term conditions as one of its key areas. The Policy Framework is relevant across a wide range of conditions and all care settings. It was developed in conjunction with the Long Term Conditions Alliance Northern Ireland, which is an umbrella body for voluntary and not for profit organisations working with and for people with long term conditions in Northern Ireland.
"My officials are working to progress the establishment of a Regional Implementation Steering Group to take forward the implementation of the Long Term Conditions Policy Framework."
