A care plan should include details of the services to be provided, who will provide them, any charges you must pay and whether you will get a direct payments or a personal budget to enable you to pay for the services you want.
It should also include information about a future review of your needs and who to contact if any problems arise.
If you don’t agree with the authority’s assessment or think the help you are offered will not meet your needs, ask for full written details about their decision, compare it with their published eligibility criteria and follow their complaints procedure.
