Care needs checklist

Different local authorities assess care needs in different ways. Some even have online self-assessment processes.

Whatever the arrangements in your area, it may help you to think about your care needs in advance and write them down.

If you are very ill, listing your care needs can be depressing but identifying them is a way of taking control so you can get the help you need and focus on other things in your life.

Before your assessment, it can be helpful to keep a diary of what you can manage each day. It will provide a record of what you can do on a good day and a bad day, to which you can refer.
