Can you get out of bed most days, some days or not at all? Are you well enough to leave the house and if so, how often?
Do you have problems moving about at home? Do you use a walking stick, Zimmer frame, wheelchair or move by holding on to someone?
Can you manage any stairs there are in your home or do you find them difficult? Would you benefit from a stair rail or stair lift?
Do you struggle to get in and out of bed? Do you need help, use a hoist, bed rail or bed raiser? If you can get out of bed, what about getting in and out of a chair? Do you need help sitting down or getting up? Do you use a chair raiser?

There may be ways to adapt your home to make living there easier. Contact the social work or social services department at your local authority. They should be in the phone book or search for local councils via the Gov.uk link (see right).
The Disability Living Foundation also has a helpline 0845 130 9177 providing information on where you can buy or hire daily living equipment and sources of funding.
